ABSTRACT First contact accessibility is the key attribute of primary healthcare and critical to the performance of the health system. It refers to the ease with which people can obtain health advice of service when they think they need it, and is best measured from the user perspective. After some paradoxical findings about accessibility in rural and urban contexts, we hypothesized that current measures do not reflect significant barriers in rural contexts, we explored barriers to access in rural and urban areas, then developed and adapted reliable sub-scales of geographical, organizational and economic accessibility that were predictive of access problems.
