CHECKLIST OF REQUIREMENTS FOR REPORT OF MARRIAGE

IMPORTANT: The Embassy will not process your application unless all of the following requirements are complied with. Processing time is approximately five (5) working days.

 Report of Marriage forms legibly and completely accomplished in quadruplicate. If obtained through the internet, kindly print four copies and accomplish accordingly.  Original and four photocopies of the Marriage Contract or Acte de Mariage of the couple.  Original and four photocopies of the first (data page) and last page of the husband’s passport.  Original and four photocopies of the first (data page) and last page of the wife’s passport.  Four passport-size photos of the husband in plain white background which are pasted onto each of the Report of Marriage forms.  Four passport-size photos of the wife in plain white background which are pasted onto each of the Report of Marriage forms. NOTE: Originals of the documents required will be returned to the applicant upon verification of the Processing Officer.  The photocopies of the documents required above should be stapled to each Report of Marriage forms as follows: o o o o

Copies of the Marriage Contract or Acte de Mariage, passport of husband, and passport of the wife should be stapled to the Report of Marriage Copy for the Applicant. Copies of the Marriage Contract or Acte de Mariage, passport of husband, and passport of the wife should be stapled to the Report of Marriage Copy for the Philippine Embassy. Copies of the Marriage Contract or Acte de Mariage, passport of husband, and passport of the wife should be stapled to the Report of Marriage Copy for OCA-CRD. Copies of the Marriage Contract or Acte de Mariage, passport of husband, and passport of the wife should be stapled to the Report of Marriage Copy for the NSO.

 Official receipt as proof of payment.

Payments should be made either in the form of cash or Mandat Cash payable to Ambassade des Philippines. Payments through credit card or in the form of checks are not accepted. SCHEDULE OF FEES: o €25 for the Report of Marriage o Additional €25 for a Delayed Report of Marriage made beyond fifteen (15) days counted from the date of marriage o Additional €7 for mailing the Applicant’s Copy of the Report of Marriage to his or her residence.
